My map is formed by a
combination of the following layers; image, simple marker overlay, and in-memory
feature layer (i.e. marker labels). When the map is exported to pdf, only the
image remains visible and the rest is not visible. Can you give me a sample
code for ASP.net application showing how to do it properly?

It looks the mainly logic is get image from map and print the image to PDF.
But if you choose the 3rd part background layer, this way maybe still don’t works. If you have this requirement, I will ask our developer about how to handle it.
